We designed this Health Passport for Torfaen Social Services for people with learning disabilities and other health problems. The information within uses illustrations to convey personal and confidential information.

The owner of the manual has sections to complete by hand such as diet, general health, immunisation, address details, care information etc. The passport is a working document and has 40 sheets of A5 paper in a ring bound mechanism. Pages can then be added for future use. The booklet has a protective Polyprop outer cover, illustrations are screen printed with one colour.
